What actually makes a software project fail, beyond bad code?

We've seen projects fail even when the dev team was technically solid. From people who've actually shipped stuff, what are the non-technical reasons projects actually fall apart?

Honestly it's rarely the code. Most failures we've seen come down to scope creep nobody re-negotiated, no single person on the client side actually owning decisions, and nobody agreeing on what 'done' even looks like before starting. Skipping a real discovery phase to save a week upfront almost always costs way more than a week later.

Biggest one for us was the client stakeholder who kept changing their mind halfway through and nobody above them ever signed off on anything. Total chaos.

So true about scope creep, we started with a tiny 6 week build and 'just one more feature' turned it into a 4 month project somehow.
